NTACHC’s Living Your Best Life Program Celebrates Five Years of Success in Diabetes and Hypertension Management

FORT WORTH, November 13, 2025 – North Texas Area Community Health Centers (NTACHC) announces the continued success of its Living Your Best Life Program (LYBL), an essential part of the National Hypertension Control Initiative (NHCI). The pivotal initiative aimed at empowering patients to manage diabetes and high blood pressure effectively has been a cornerstone of NTACHC’s commitment to improving patient health since 2020. LYBL has demonstrated exceptional results in controlling hypertension through innovative strategies and dedicated care to address diverse patient needs.

NTACHC looks to address chronic health conditions with a comprehensive and holistic approach. Through LYBL, patients are provided with essential education and support on diabetes and hypertension management. This includes personalized diabetes education, prevention strategies, and guidance on healthy eating and exercise habits. Additionally, the program focuses on managing high blood pressure by teaching patients about hypertension control and promoting crucial lifestyle changes. Patients’ providers work with LYBL staff and the pharmacy team, ensuring that they are receiving the highest care possible. 

Since joining in 2020, NTACHC has expanded its efforts by participating in the National Hypertension Control Initiative (NHCI), a collaborative project led by the American Heart Association and HRSA. This partnership enabled NTACHC to incorporate remote patient monitoring (RPM) into the LYBL program. Utilizing Bluetooth-enabled blood pressure monitors, RPM has proven to be a game-changer for patients with uncontrolled hypertension.

The results speak volumes: nearly 80% of patients enrolled in RPM are now managing their blood pressure effectively. Currently, NTACHC has more than 350 patients actively engaged in RPM, with more than 1,170 patients having benefited from the program since its launch. This remarkable achievement demonstrates the effectiveness of RPM in enhancing patient outcomes and underscores NTACHC’s dedication to patient-centered care.

NTACHC’s “Living Your Best Life” program also showcases the success of integrating Self-Measured Blood Pressure (SMBP) monitoring into hypertension management. By adopting SMBP technology, NTACHC has improved patient engagement and hypertension control. Their phased approach, from manual to fully integrated technology, has enabled real-time data transmission and personalized care, leading to better health outcomes. The use of both technological and manual options ensures inclusivity and effective data collection for their patients. 

Dr. Patricia Rodriguez, NTACHC Chief Medical Officer, expresses gratitude for the exceptional efforts of the NTACHC team: “Our Special Programs staff, Carmen Lavarreda, LVN, and Gaby Hernandez, CHW, have done a phenomenal job in implementing and executing the RPM initiative. Their dedication, combined with the high-quality care from our providers, has significantly improved patient outcomes and knowledge. I extend my heartfelt thanks to Carmen, Gaby, their fearless leader Dionne Jackson, and our adult providers for their outstanding work in keeping our hypertensive patients healthy.”

In August 2025, LYBL completed its first Facial Recognition CAREMINDr set-up with a patient, allowing patients to take their own blood pressure. In a test at the center, the LYBL nurse walked the patient through, and she followed up with a manual blood pressure reading that was spot-on accurate. 

NTACHC’s success has garnered national attention, with the organization being featured in the NHCI Data Integration Roadmap shared by HRSA. This recognition highlights NTACHC’s role as a model for other health centers seeking to implement RPM and enhance health outcomes across the country. 

LYBL also achieved a significant milestone of accreditation by the Association of Diabetes Care & Education Specialists. Accreditation represents a high level of quality and service to the community, and the ability to better meet the needs of Medicare recipients and others affected by diabetes in the Tarrant County area.
