We are a community family medical practice that offers a full-range of primary and preventative healthcare services to our patients. We offer a coordinated team-based approach to healthcare, in which a team of medical professionals work together to provide for your primary health care needs (acute, chronic and preventative) and assist in arranging appropriate care with other qualified professionals and/or specialists to ensure your continuity of care. Our teams at NTACHC include physicians, physician assistants, nurses, medical assistant, pharmacists, social workers, and educators. This model results in more personalized, coordinated, effective and efficient care.
NTACHC works closely with you and your family to understand and respect your unique needs, culture, values, and preferences. We actively support you in learning to manage and organize your own care at the level you so choose.
